Position Summary:  Responsible for developing and maintaining client relations and intake of direct referrals made to the agency.

The primary job duties of the Intake Coordinator include, but are not limited to:
 Working with physicians and hospitals to identify best Home Care solutions for their patients.  Arrange Durable Medical Equipment, Oxygen, etc. with outside vendors. Confirm insurance coverage on all referrals. Work in conjunction with Insurance Specialists to obtain required verification’s and or authorizations. Document according to department, agency & regulatory guidelines. Actively participate in department and agency meetings with solutions-oriented contributions. Demonstrate ability to work in fast paced environment while maintaining attention to detail. Timely and accurate creation & processing of incoming community, hospital & rehab referrals across NY state. Demonstrate effective communication with referral sources and  other departments and any other duties as assigned.

Minimum Qualifications: LPN preferred but not required.  Strong organization and computer skills required. Excellent customer service/communication skills essential. Attention to detail. Acute care or Home Care experience preferred.

Salary: $18-$24/hour, Depending on Experience.

Days/Hours: Tuesday through Saturday, 8:30 AM – 5 PM and Sunday through Thursday 8:30 AM to 5:00 PM

This will include job duties such as: overseeing med passes, training direct support staff, reviewing and scheduling preventative care services, and accompanying residents to medical appointments.  The right candidate will work well as part of an interdisciplinary team, will possess critical thinking skills, and will be able to provide basic nursing assessments along with the creation of care plans.

Minimum Qualifications: Bachelor’s degree in Nursing required. You must have a valid NYS Nursing license. 3 years of related work experience required. Medical/Utilization Review experience preferred. Medicare Set-Aside Arrangement experience highly preferred. Experience with ICD 9/10 Codes, CPT Codes, and exposure to applying actual costs/workers compensation fee schedules preferred. Knowledge in Medicare Coverage guidelines, pharmacology, clinical practice guidelines, Utilization Review Standards and practices, state-specific workers’ compensation guidelines and pricing structure helpful.

Minimum Qualifications:

  • Bachelor’s degree from an accredited college or university in Nursing required
  • At least 2 or more years’ experience of clinical experience, previous chart review experience a plus!
  • Knowledge of Medicare programs, particularly coverage and payment rules
  • Basic experience with Microsoft Office (Word and Excel) and the ability to research utilizing the Internet preferred, including web-based portal
  • RAC or RVC review experience strongly preferred
