As a Outreach Specialist, you will work to build and maintain relationships with Managed Care and Community Based Organizations to identify and enroll eligible individuals in the Health Home program. This is a Direct Hire position offering competitive salary and great benefits! Hours are full time, Monday through Friday, 8:00AM-5:00PM.
An Associate’s degree in Social Work, Human Services, Counseling or a related field and/or 1-2 years’ experience in community health is required. There is significant travel throughout Monroe County, so reliable transportation and a valid driver’s license are both required.
