We are conducting a search for RN’s with two years of Case Management or community health exp. preferred to work in the community performing UAS assessments.  This is an exciting opportunity to join a managed care organization that empowers individuals to create healthy habits that last a lifetime.  In this role you will assist in evaluating patients and perform case management.  This is a non-direct care role.  Extensive travel throughout the county is needed.

These positions exist to lower costs and improve outcomes for the under-served population. You will be responsible for case managing members and assisting them to receive needed services.  In this role you will work within the community performing UAS assessments.  These assessments are used to facilitate the linkage of the patients to those services they are eligible to receive. You will monitor delivery of services and follow-up with members, caregivers or providers through in-person visits and telephone contact. You will assist in coordinating the development of appropriate discharge plans through collaboration with physicians, discharge planners, members and their families. You will assist members with filing and resolving complaints and appeals.

Minimum Qualifications: Three years of care management experience preferred.  Home health, discharge planning or long term care experience is preferred.  A valid RN and NYS driver’s license are required.

Days/ Hours: This position is 40 hours, full-time, Monday through Friday.

Salary: $31-40.00 per hour, DOE
