We are conducting a search for RN’s with prior Case Management or community health experience to work in the community performing UAS assessments. This is an exciting opportunity to join a managed care organization that empowers individuals to create health habits that last a lifetime.

You will be responsible for traveling throughout the county to conduct UAS Assessments within patient homes. We have one opening with travel throughout Erie County and another opening with travel throughout Niagara County. Mileage is reimbursed.

UAS assessments are used to facilitate the linkage of the patients to those services they are eligible to receive. Your primary responsibility will be to complete assessments with members, caregivers, or providers to obtain information regarding client status, support system, and need for services for care plan development. Other job duties include but are not limited to: assisting in coordinating the development of appropriate discharge plans through collaboration with physicians, discharge planners, members and their families, assisting members with filing and resolving complaints and appeals, monitoring delivery of services, and following-up with members, caregivers, or providers through in person visits and telephonic contact.

Minimum Qualifications: Three years of care management experience preferred. Home health, discharge planning or long-term care experience is preferred. A valid RN and NYS driver’s license are required.

Days/ Hours: This position is 40 hours, full-time, Monday through Friday.

Salary: $31-40.00 per hour, DOE.
