As a Medical Case Manager, you will strive to improve clinical outcomes and provide cost-effective services for patients. By using a combination of research, investigation, and patient health education, you will provide linkage to appropriate community resources; make referrals to appropriate care services, and communicate with providers in order to improve care outcomes. Your case load will include a variety of backgrounds including the uninsured, the Medicaid population, and other vulnerable patients. The majority of the work is telephonic. You may be asked to travel to client’s homes to assess their care; however, there is no direct patient care.
Schedule: Full Time, Monday to Friday, 8:00AM-5:00PM

” A RN, LCSW, LMHC, LCAT or PhD will call be considered
” A minimum of three years clinical and behavioral health experience is required
” Excellent written and verbal communication skills are required
” Previous case management or community health experience is an asset
