As a Medical Case Manager, you will strive to improve clinical outcomes and provide cost-effective services for patients. By using a combination of research, investigation, and patient health education, you will provide linkage to appropriate community resources; make referrals to appropriate care services, and communicate with providers in order to improve care outcomes. Your case load will include a variety of backgrounds including the uninsured, the Medicaid population, and other vulnerable patients.
The majority of the work is telephonic, and after approximately 2 weeks of training in Rochester (mileage and hotel reimbursed), you will have the comfort and convenience of working out of your home! You may be asked to travel to client’s homes to assess their care, on occasion; however, there is no direct patient care.
Schedule: Full Time, Monday to Friday, 8:00AM-5:00PM
” The following clinical licensures will all be considered: RN, LCSW, LCAT, LMHC
” A minimum of three years clinical and behavioral health experience is required
” Excellent written and verbal communication skills are required
” Previous case management or community health experience is an asset
