The minimum qualifications include one year of acute care experience, two is preferred. With the target of five visits a day, you will case manage and co-ordinate care for patients with a variety of illnesses. Some visits will need vitals and assessment; others will require the most complex nursing interventions you can provide. Your above-average organizational skills make it possible for you to deliver care in the community efficiently and with accuracy.
