Access, document, and manage acute medical care needs of assigned patients in collaboration/consultation with the attending physician. Completes discharge summaries and instructions for assigned patients. Conduct daily rounds. Assess, document, and manage chronic medical care needs of assigned patients in collaboration with the attending physician. Collaborate with the attending physician and other members of the multidisciplinary team to devise a plan of patient care. Monitor patients during diagnosis and therapy; maintain a record of patients' progress. Coordinate interdisciplinary care and support Patient-Family centered care for assigned patients. Provide consultation, advice to floor staff in assigned areas. Provides counsel, support and education to residents and their families in collaboration with the health care team. There is off hours shared telephone call responsibility of ~5 days per month. Monitor team office staff action screens daily. Assist the DON with screening, hiring, orientating and terminating of field and team office staff. Review infection control reports for all clients and provide/document all follow-up related to infection control reports.
