RN - PER DIEM HOME VISITS
RN - PER DIEM HOME VISITS RN’s several opportunities needed for private home visits. Responsible for responding to calls from triage staff members regarding home care patients who require weekend visits and for making appropriate clinical interventions. Must be able to commit to Orientation 3 to 4 consecutive weekends or 6 weeks ft days plus shadowing up to 2 more weeks before taking per diem assignments. The preference is for candidates who will commit to an eight-hour shift. Per Diem rates: Weekdays $35/visit, evening/visit: $40/visit; Weekend visit: $40/visit, weekend open case: $90.00/visit, Weekday evening open: $80/open; weekday opening: $75/open. There is a current search for a per diem position every Friday evenings 5pm-8pm; in hospice there is an every other weekend position with these per visit rates in a hospital setting. Community Health Nurse Specialist Roles
Direct hire. RN Community Health Nurse to visit both adult and pediatric patients in the southern Monroe county area. RN Case Manager: Home visits to hospice patients, staff oversight, pain and symptom management, Western Monroe County position. Part-time IV Specialist: evenings (during the week) 2-10 - SOUND IV skills required! Part-time Hospice Nurse, scheduled every other weekend with the hospice program. RN CASE MANAGER
RN Case Manager for home care and/or hospice with some recent Med Surg exp. There are 4 to 5 RN and they have and LPN who will do 6 to 8 visits a day for them. 40 hours per week, 8 per day (usually 8-4:30pm with some flex). The on-call expectation currently is that RN would take week-end on-call or work rotation every 3-4 weekends and two weekdays of either on-call or work rotation very 4-5 weeks. To cover all four Counties- Yates, Ontario, Senaca, Wayne. Qualifications: Possession of a current NYS Registered Nurse licensure; minimum of 6 months to one year medical/surgical experience (hospital setting preferred) for home care nursing; medical/surgical experience or oncology experience for Hospice nursing; ability to communicate effectively, work independently and maintain effective interpersonal relationships. Full-time Monday – Friday 8am-5pm Clinical Educator. HMO setting. Summary: Responsible for developing and implementing a robust, continuing medical education program for clinical staff to improve productivity and effectiveness of utilization management and case/disease management programs. Responsible for managing an in-depth, structured, new employee orientation program for clinicians in the Health Resource Management Department. Designs and conducts training modules and study sessions for the organization’s clinical certification programs. Responsible for training clinicians for Certified Professional Utilization Review certification program. POSITION QUALIFICATIONS: Minimum Education: Bachelor’s degree in nursing, health education, or instructional development. Masters degree preferred. RN Home Care Infusion Educator
Home Care Infusion Educator for growing home care agency. Will be responsible for the development and evaluation of Infusion programs and staff education in the care of Infusion clients. This responsibility may include participation in the screening of Infusion specialist applicants, orientation, education and evaluation of Infusion specialist staff, ensuring that quality Infusion care is delivered efficiently. Ophthalmic Tech M-F 40/hrs with occasional Saturday (1 out of 5), some evenings some travel to their other location, this position is in Brighton. Clinical medical background, may be a nurse of Ophthalmology Tech. Duties: Pre- exam work up of patients for optometrists, refraction of patients, visual acuity testing, taking medical histories, eyeglass prescription analysis, running visual fields, general assistant with contact department and scribing (will be taking notes during pt visit), support the Dr., some phones. Must have good numerical skills. Must be very bright, hard-working and reliable. Ophthalmology exp. preferred, salary: $14/hr to $19/hr depending on the exp.- they want the best person available!



LPN- COMMUNITY HEALTH NURSING!
LPN for 2 12-hour shifts, every Saturday and Sunday. In this role you would be responsible for the delivery of nursing services to assigned patients. The duties and responsibilities for the care to be delivered by the LPN are identified and defined by the Supervising RN. The LPN most often is responsible for administering specific nursing interventions, such as dressing changes, catheter care, injections and may also be involved in patient/family teaching.

Pediatric Nurse Practitioner for hospice and palliative care, part-time or per diem in Monroe County. In this role you would be primarily responsible for providing consultation to both community health care providers and institutional health care providers regarding both palliative care and continuity of care. The PNP would also be responsible for providing information on palliative care options available to assist families in the decision making process as desired. RN Clinical Review- Utilization Management

RN Chart Review Officer
Long term temp contract position, $25/hr., requiring strong knowledge of Medicare Regulations, strong writing and verbal communication skills. Ability to type with strong computer skills. Work with a team of physicians, nurses and attorneys within a flexible weekday schedule to assist patients with denied Medicare claims. Acquires all necessary information involving cases and renders a determination based on the information. Communicates the determination in a clearly written letter and ensures that the letter is sent to all appropriate parties as required by the specific client contract. Collects and ensures that all data collected in a case file is accurate and enters and maintains data to related appeal computer tracking system. Maintains the case file in appropriate order so that others (i.e., re-openings, ALJ) can easily understand the file. Performs other special projects not related to a specific case such as general legal research, general medical research, drafting proposal sections, or acting as a liaison for a specific project, when necessary. Meets or exceeds all standards established for this position. Demonstrates teamwork and promotes positive company relations. Performs other duties as may be assigned by management.
